Loss information must be complete and accurate and it must include currently valued loss runs for a minimum of seven years.
Information on you client’s current insurance coverage is critical. If they don’t currently have a carrier, we need to know why.
Current Declaration page or certificate
Physician’s Curriculum Vitae (CV)
Copy of all advertising, including yellow page ads
Copy of applicant’s business letterhead

With some basic information we can usually provide an indication of availability and pricing over the phone. To get the best options and pricing, within 45 days of the desired effective date, we need a complete application form with all data filled out. The more information you can supply, the better.
