        Non Standard Physicians

  • Loss information must be complete and accurate and it must include currently valued loss runs for a minimum of seven years.
  • Information on you client’s current insurance coverage is critical. If they don’t currently have a carrier, we need to know why.
  • Current Declaration page or certificate
  • Physician’s Curriculum Vitae (CV)
  • Copy of all advertising, including yellow page ads
  • Copy of applicant’s business letterhead
  • Copy of state medical license


        Medical Facilities & Hospitals

  • Loss information must be complete and accurate and must include currently valued loss runs for a minimum of five years.
  • Information on you client’s current insurance coverage is critical. If they don’t currently have a carrier, we need to know why.
  • Audited financial statements
  • Risk management guidelines and procedures

  • Detailed exposure data
